Every athletic participant must have a physical, payment and registration completed at Blevins in order to practice or compete. The physical forms are valid for a year from the examination date. It is advisable to arrange physicals well in advance of the sport in which your student is interested. Please note: Athletic Physical forms are available on the PSD and Blevins websites, or in the Blevins Front Office. Physical forms are not available in the physician’s offices. Softball season dates are March 4th through April 27th. First day of practice is Monday, March 4th. Fee for Softball is $105 and is paid before athletes attend their first practice. Girls who are interested in playing Softball need to sign up on the clipboards in the Auditeria when announcements are made regarding the start of Softball.
You must complete an online registration through SportsEngine or a 2018-19 Middle School Athletic Participation Permission and Release form and a 2018-19 Student Athlete Medical Information form which must be completed, signed and submitted to Mrs. Koepp in the Blevins Front Office. If you have already submitted these forms for the 2018-19 school year, there is no need to resubmit them.
You must have a current Middle School Physician Certification of Student Fitness for Athletic Participation on file in the Front Office. This form must be submitted to Mrs. Koepp regardless of registering online or submitting a paper copy of your registration. The Physician Certification is good for 365 days from the date of the exam.
A $105 athletic fee (per sport) must be paid or waived prior to tryouts or participation. The athletic fee for in-district students is $105 and the fee for home-based, charter, and private school students is $157 per sport. Payments can be made online through SchoolPay or you may submit cash, check or pay with a credit card in the Blevins Front Office. All credit card transactions are charged a non-refundable fee. If your student qualifies for Meal Benefits with eligibility for a waiver, the 2018-19 Permission to Share form must be completed and submitted through your ParentVue account.
Once your student has completed the payment and registration process, they will be issued a Silver Card that is given to their coach. There are no exceptions. Cards will be issued to athletes on the first day of practice.
Students are encouraged to not wait until March to obtain a sports physical, register and pay for their sport.
Last minute submissions could delay the process and may result in your student not be issued a Silver Card until the following day.
