Sports Engine (Online Athletic Registration)
Every athletic participant must have a physical, payment and registration completed in order to practice and compete. The physical forms are valid for a year from the examination date at the doctor's office not the date the form is signed by the physician. It is advisable to arrange physicals well in advance of the sport in which your student is interested. Please note: Athletic Physical forms are available on the PSD website or in the Blevins Office. Registering online through Sports Engine is the preferred method. Physical forms are not available in the physician’s offices. If you complete the forms through Sports Engine, the only form needed to be delivered to the Blevins Office is the 2019-20 Middle School Physician Certification of Student Fitness for Athletic Participation.
2019-2020 Athletic Fees:
Required Payment: $110/sport per athlete. Payment can be made in cash, check or credit card to Mrs. Koepp, Blevins Bookkeeper or online through School Pay.
For assistance having fees waived, you must have (1) applied each year for Free and Reduced Meal Benefits through PSD Food Service; (2) upon approval for meal benefits, access your ParentVue account. If you do not yet have an access code for ParentVue, please contact Blevins Student Services, 970-488-4008. In ParentVue, click on Forms, Permission to Share form and click on each fee to be waived. Submit your form. This submission requires 24 hours to process. (3) Through School Pay, open your cart and click on each fee that you want waived (these will match the items from your Permission to Share form). Process and complete the transaction. Your shopping cart should zero out.
Required Silver Card: All athletes present to their coach a Silver Card, obtained from Mrs. Koepp after completing paperwork and payment. The Silver Card signifies that all requirements have been met and they are then Practice-Ready.
Athletic and School Insurance Information:
PSD insurance does not cover student injuries due to accidents at school or during school-sponsored activities, such as field trips. A voluntary accident insurance program is available through Nationwide and serviced by K & K Insurance Group. If you don’t have other insurance, this plan may be a resource to consider. Even if you have other coverage, this plan can help fill expensive “gaps” caused by deductibles and co-pays. All athletes are required to have insurance in order to participate in athletics. The Health and Wellness Center at Centennial High School (2 blocks east of College Avenue and Laurel Street) offers sports physicals/well child exams June through August. The Center can bill insurance and no child is ever turned away for inability to pay. For more information visit www.hwcenter.org or call their office at 970-488-4950.
Another source for sports physicals is ChristClinic. There are two locations. Physicals are by appointment only and are free. Faith Church, 3920 S. Shields, 970-481-2390, Monday evenings only, 6:00 to 8:30 PM. http://www.faithchurchfortcollins,org. Mountain View Community Church, 328 Remington St., 970-481-2390, First Saturday morning of the month, 9:00-11:00 AM and third Friday of the month, 6:00-8:30 PM. http://www.mvcchurch.org.
